Participant Consent Notice

Your organisation has engaged Sensewise, an independent consulting firm, to conduct an organisational culture diagnostic. This survey is part of that process.

Your responses to rating scale questions, scenario questions, and optional open-ended questions about your experience at work. We do not collect your name, employee ID, or any information that directly identifies you to your employer.

When you start this survey, we save a temporary resume key in this browser. If you close the browser before finishing, you can return to the same device and continue from where you left off. The resume key is a random code — it contains no personal information and cannot identify you. It expires automatically after 7 days or when you submit the survey, whichever comes first. After it expires, it stops working entirely.

If you are using a shared or public computer: we recommend completing the survey in one sitting. If that is not possible, clear your browser's local storage or site data after each session to remove the resume key from that device.

Your individual responses go directly to Sensewise — not to your employer. Your employer receives only aggregated results (group averages and patterns). No individual response is ever shared with your employer in an identifiable form.

All data is stored securely on servers in India. Your partial responses are deleted automatically after 7 days if you do not complete the survey. Completed responses are retained for the duration of the diagnostic engagement and deleted within 12 months of its conclusion.

You may withdraw your consent at any time by contacting privacy@sensewise.in. Withdrawing consent will delete your responses and invalidate your resume key. It will not affect your employment. You may also request access to your data or ask us to correct or delete it.

Completing this survey is entirely your choice. There are no consequences for declining or for leaving any question blank.
